What to Include in the Affidavit
- Parent/Guardian legal name and mailing address
- List of child(ren) being homeschooled, including grade level and date of birth
- Academic year start and end dates, and planned number of instructional days
- Name and relationship of the primary instructor
- Optional curriculum provider, approach, or course of study
- Statement confirming you will maintain required records (attendance, coursework, instructor information)
- School name (if applicable) and address where records will be kept
- Custodian of records (name of responsible adult)
- Statement regarding coeducational status (if applicable)
- Statement confirming criminal background checks have been obtained for all instructors, if required by local law
