What to Include in the Affidavit

  • Parent/Guardian legal name and mailing address
  • List of child(ren) being homeschooled, including grade level and date of birth
  • Academic year start and end dates, and planned number of instructional days
  • Name and relationship of the primary instructor
  • Optional curriculum provider, approach, or course of study
  • Statement confirming you will maintain required records (attendance, coursework, instructor information)
  • School name (if applicable) and address where records will be kept
  • Custodian of records (name of responsible adult)
  • Statement regarding coeducational status (if applicable)
  • Statement confirming criminal background checks have been obtained for all instructors, if required by local law
